Ordering and Payment Options

Cindy Jones Associates accepts Visa and Master Card. Orders can also be made with hospital purchase orders. Phone or fax orders are also accepted. Hospital purchase orders are accepted via postal mail as well. Whatever method you choose to purchase from us, just follow the instructions after adding the item(s) to the shopping cart system.

At any time, you can press the "View Your Shopping Cart" button to review what you have in your cart or the "Go To Checkout Now" button to complete your order.

You will be taken to the shopping cart system where your order will be submitted via secure server. We will confirm your order via email. The shopping cart system will also let you print out your order form and then phone (734) 769-6536 or fax (734) 769-8556 it to us.

Shipping & Handling:

$8.00 will be added for each set of Hospital Gift Shop Mailing Labels.

Sales Tax:

6% sales tax will be added for Michigan residents.

Canadian orders will be in US dollars.
